Girls flag football is a fully sanctioned varsity championship sport in North Carolina. On May 6, 2026, the Board of Directors of the North Carolina High School Athletic Association (NCHSAA) voted to sanction girls flag football, with competition beginning in fall 2026 and the inaugural NCHSAA state championship scheduled for late 2026. The NCHSAA is the sport's governing body statewide, aligning play with NFHS rules standards.

The sport's growth traces directly to the Carolina Panthers, who launched the first North Carolina pilot in 2022 with 19 schools.
